A witness’s words are worth more in writing.

An independent witness can make all the difference in a dog bite case — but only if their account is captured before they forget or move on. This free form records exactly what they saw, in their own words.

Why a witness statement matters

A neutral third party can confirm your version — and quietly dismantle the owner’s.

Dog owners and their insurers often claim the victim provoked the dog, was trespassing, or exaggerated what happened. A witness who saw the dog loose, unprovoked, or acting aggressively can corroborate your account and shut those defenses down. The catch is timing: memories blur within days and people move away or lose interest. Capturing a clear, signed statement now preserves it while it’s reliable.

What the form captures

✓The witness’s name and contact information
✓Where they were and what they saw, in their own words
✓Whether the dog was leashed, contained, or running loose
✓Whether anyone provoked the dog
✓How the owner reacted afterward
✓The witness’s signature and the date

Tip: have the witness fill it out themselves and sign it. A statement in their own handwriting or typed by them carries more weight than one written for them.
